We want you to feel like you are part of our family and want you to rest assured that the care we provide to you will be the same that we provide to our own families.
Patient Information Forms
For your convenience, we’ve provided the patient information forms you’ll need to fill out before your first visit – Adult Patient Information, and Child Registration. You can fill out the forms at home and bring them with you to the office, or you can send them to us ahead of time by fax (864-233-6314). If you don’t have time to fill out the forms at home, we recommend you arrive 15 minutes early to your appointment so you have time to fill them out that day.
You can also take a look at our HIPPA forms. This is for your reference only. We’ll ask you to sign a copy in the office on the day of your appointment.
